Symptome eines Handekzems

Typische Symptome für eine Handdermatitis sind:

• starker Juckreiz
• aufgerissene Fingerspitzen
• Schuppung der Haut
• trockene Haut, die verhornen und in der Folge aufreißen kann
• Bläschenbildung

Häufig sind von einer Dermatitis die Handinnenflächen und die Innenseiten der Finger betroffen. Aber auch der Handrücken sowie die Außenflächen der Finger können betroffen sein.

Durch ein Handekzem kann der betroffene Patient stark in seiner Lebensqualität eingeschränkt sein. Zum einen trauen sich viele Patienten nicht mehr, ihre Hände „vorzuzeigen“, aus Angst, vermeintlich abstoßend oder unappetitlich zu wirken. Zum anderen können Berührungen, das Zugreifen oder Anfassen äußerst unangenehm werden, da die trockene und verhornte Haut schnell einreißen kann. Schmerzhafte Furchen können dabei entstehen. Der starke Juckreiz tut sein übriges dazu.

Diagnose und Differentialdiagnose

Aufgrund der vielen möglichen Ursachen für die Entstehung von Handekzemen gibt es nicht eine Therapie für alle Fälle. Nicht nur die Ursache, sondern auch die Erscheinungsform, der Ort der Ausprägung und der Schweregrad müssen bei der Wahl der geeigneten Behandlungs methode beachtet werden. Zudem muss zwischen akuten und chronischen Handekzemen unterschieden werden.

In der Regel erfolgt die Behandlung mit äußerlich anzuwendenden Wirkstoffen (topische Therapie). Bei nicht Ansprechen auf eine rein äußerliche Behandlung, bei schwerer Ausprägung der Handekzeme kann zusätzlich eine Phototherapie oder eine systemische (Tabletten-) Therapie durchgeführt werden.
Leichte Handekzeme sollten schnell und konsequent therapiert werden, um einer Chronifizierung und der Entwicklung eines Kontaktekzems über die gestörte Hautbarriere entgegenzuwirken. Chronische Handekzeme mit Nachweis von Kontaktallergien sind oft nur sehr schwer zu therapieren.

Für eine effektive Behandlung ist wichtig, dass auslösende Faktoren wie Allergene, reizende Stoffe und ungünstige Verhaltensweisen wie häufiges Händewaschen konsequent vermieden werden.

Basistherapie von Handekzemen

Jede Therapie von Handekzemen bedarf einer Basistherapie, die einen äußerlichen Hautschutz und eine regelmäßige Hautpflege beinhalten. Hautschutzpräparate können als ein „flüssiger Handschuh“ beschrieben werden, der die Haut versiegelt und hierdurch die Hautbarriere vor aggressiven Substanzen schützt, Das Präparat, z.B. Excipial® Protect ist am sinnvollsten morgens, mittags und spätnachmittags einzusetzen.

Zur Austrocknung von stark juckenden Bläschen können außerdem synthetische Gerbstoffe eingesetzt werden.

Medikamentöse topische Therapien

Die Therapie der ersten Wahl zur Behandlung von Handekzemen ist der Einsatz von antientzündlich wirkenden topischen Glukokortikosteroiden. Abhängig vom Schweregrad der Er krankung werden Substanzen unterschiedlicher Wirkstoffklassen eingesetzt, die ver schieden stark wirken. Aufgrund der Nebenwirkungen, zu denen ein Ausdünnen der Haut (Atrophie) und eine Schädigung der Hautbarriere zählen, können topische Glukokortiko steriode nicht ununterbrochen über lange Zeiträume angewendet werden.
