Maculopapular Rash

A macule is a flat blemish or discoloration that measures less than 1 cm. A papule is as elevated lesion measuring less than 1 cm. Combining the two terms, a maculopapular rash is as a smooth skin rash or redness covered by elevated bumps [1, 2, 3].

Other names for it are morbilliform eruption or exanthematous eruption (exanthema).

Due to the presence of inflammation, maculopapular rash appears as reddened or erythematous. The distribution may be concentrated on a certain area of the body or it can be generalized. It may be acute, subacute, or chronic. Acute maculopapular rash lasts less than 4 weeks, 4-8 weeks for subacute, and it is considered chronic if the rash lasts for more than 8 weeks.

The occurrence of maculopapular rash for children is generally due to viral infection. For the adults, drugs and allergies are often the culprit.

Maculopapular rash is associated with a lot of factors. In the absence of fever, maculopapular rash is not necessarily a problem. But if the patient is febrile and has some other signs and symptoms, maculopapular rash might be a sign of a serious disease so do not ignore it [4].

Das häufigste Symptom einer HIV- Infektion ist das Auftreten von teilweise sehr hohem Fieber. Rund 90 Prozent aller Infizierten entwickeln in den ersten Monaten nach Ansteckung Fieberschübe. Auch auffällige, tastbare Schwellungen der Lymphknoten sind in der ersten Zeit nach der HIV- Infektion keine Seltenheit. Es entstehen Verdickungen der Lymphknoten im Nacken-, Achsel- und/ oder Hals- Bereich.

Ungefähr 70 Prozent aller HIV- Kranken klagen über starke Rachenentzündungen und einen stark ausgeprägten Hautausschlag (sogenanntes HIV- Exanthem, Hautausschlag HIV). Der Hautausschlag bei HIV weißt eine kleinfleckige- knotige Struktur auf, man spricht von einem makulopapulösem Hautausschlag, der mit dem während einer Masern oder Scharlachinfektion vergleichbar ist. Der Hautausschlag durch HIV tritt in den meisten Fällen im Bereich des Gesichtes und des Körperstamms auf, bei manchen Infizierten lassen sich die Papeln jedoch auch an den Extremitäten (Armen und Beinen) auffinden. Der eben beschriebene Hautausschlag spricht am ehesten für eine erst kürzlich erworbene HIV- Infektion (sogenanntes Stadium A) und zeichnet sich durch ein leichtes Jucken der betroffenen Körperregionen aus. Wenige Tage (meist schon 24 bis 48 Stunden) nach dem Auftreten des makulopapulösen Hausausschlages verschwinden die Papeln plötzlich, zu diesem Zeitpunkt geht auch der Juckreiz zunehmend zurück. In seltenen Fällen entstehen anschließend geschwürartige Entzündungen (Ulzera) im Bereich der Mundschleimhaut.

Diagnosis

If you have a maculopapular rash, your healthcare provider will ask you a series of questions about:

• Your medical history
• Your vaccination status
• Any allergies you have
• Medications you take
• Any other symptoms (such as fever or fatigues)

You will likely also be asked if you've been exposed to someone with a rash-causing illness or if you have traveled to a place where certain viruses are common.

Your provider will perform a full physical examination that includes a close look at your skin. Sometimes the characteristics of a maculopapular rash can help identify the cause.

Blood or urine tests can help with diagnosis. In some cases, a biopsy (a small skin sample taken for examination under a microscope) can also be helpful.

Maculopapular eruptions are often nonspecific, meaning that a precise diagnosis can't be made by simply identifying the rash as maculopapular. A careful medical history and a thorough examination are important steps toward determining the cause and developing a treatment strategy.
