Comparing Nummular Eczema vs. Ringworm Symptoms

While ringworm usually shows up as a couple of circular rashes, nummular eczema often has multiple patches of circular rashes. They tend to show up on skin that is very dry, while ringworm can happen anywhere.

In general, nummular eczema is also accompanied by more symptoms than ringworm. People with nummular eczema may experience pain and burning, while people with ringworm may not.

Frequent Nummular Eczema Symptoms

Differential Diagnosis

Atopic dermatitis: Distinguishing idiopathic nummular eczema from atopic dermatitis with primarily nummular lesions requires the application of validated diagnostic criteria.

Stasis dermatitis: This condition is often associated with findings of venous insufficiency, including edema, varicose veins, and atrophic hypopigmented scars (atrophie blanche). Lesions consist of erythematous patches that may progress to scaly plaques involving the ankle and distal lower leg. Plaques may be exudative, lichenified, or superimposed on varicosities.

Asteatotic eczema: Also known as eczema craquelé, this condition appears as diffuse erythema and fine-scale, with small irregular fissures and cracks, most commonly on the bilateral lower legs.

Fixed drug eruption: This condition presents with 1 or multiple well-circumscribed circular red to brown patches or edematous plaques that recur in the exact location when the patient is exposed to the implicated drug.

Erythema annulare centrifugum: This condition appears as arcuate or round plaques with central clearing with a trailing scale at the outermost edge.

Bullous pemphigoid: This condition can present with nonspecific, pruritic, eczematous lesions for a prolonged period before the appearance of classic blisters.

Tinea corporis: This condition presents with one or multiple circular erythematous scaly plaques with central clearing. A potassium hydroxide (KOH) preparation will show the segmented hyphae and arthrospores characteristic of all dermatophyte infections.

Majocchi granuloma: This condition demonstrates multiple perifollicular pustules coalescing to form erythematous scaly plaques.

Impetigo: This condition presents as "honey-colored" or golden-crusted papules and plaques with an erythematous base.

Secondary syphilis: This condition appears as diffuse pink, red, violaceous, or brown papules and plaques with an overlying scale that often involves the palms and soles.

How do dermatologists treat nummular eczema?

The main goals of treatment are to clear your skin and ease your discomfort. To do this, your treatment plan may include treatment designed to:

Hydrate your skin: It’s likely that you have extremely dry skin. To add moisture to your skin, which will help heal your skin, you may need to:

  • Take a daily bath or shower up to 20 minutes long in lukewarm water.
  • Add bath oil to your daily bath. Bath oil can make your bathtub and skin slippery. Use caution when adding bath oil to a bath so that you do not fall.
  • Apply moisturizer as instructed throughout the day, which includes applying it to damp skin within minutes of bathing.
  • Use a humidifier in your bedroom, and keep the room cool and moist.

Moisturizer plays a key role in healing your skin. Your dermatologist may recommend a hypoallergenic, fragrance-free moisturizing cream or ointment instead of a lotion. Creams and ointments do a better job of trapping water in your skin than do lotions. Having more water in your skin can help you heal faster.

Avoid irritating your sensitive skin: Dry, sensitive skin is more likely to have frequent flare-ups of nummular eczema. For this reason, your treatment plan may include instructions that can help you avoid irritating your skin.

You may need to:

  • Use only a mild, non-drying cleanser to wash your skin, applying it only to skin that needs washing, like the armpits, feet, and groin.
  • Wear loose-fitting cotton clothing.
  • Avoid sitting next to a fireplace, heater, or other source of heat.
  • Use a humidifier in your bedroom so that you can rest in a cool, moist place.
