When Does Lichen Sclerosus Require Surgical Treatment?

Surgery is regarded as a second-line therapy for lichen sclerosus in both women and men.

In women, surgical treatment for lichen sclerosus may be required for the following circumstances:

• Correct scarring from lichen sclerosus that interferes with the normal function of the vulva, urinary system, and/or cosmetic appearance of the genital area
• Treat vulvar intraepithelial neoplasia (changes in the epithelial cells in the top or surface layer of skin that cover the vulva, often a precursor of cancer) or malignant tumor

In men, surgical treatment is reserved for circumcision in uncircumcised men when very strong topical steroids and other first-line treatments are ineffective.

Second-Line Therapies

While not regarded as first-line treatments, research indicates that the following therapies have proven to relieve some symptoms of lichen sclerosus:

Diagnose

• Zirkumskripte Sklerodermie (Guttata-Typus): kein genitaler oder analer Befall. An Rumpf und Extremiäten plaqueförmig. Oberfläche nicht pergamentartig kräuselbar.
• Lichen planus: gleichmäßiger livider Farbton, meist deutlich tastbare Erhöhungen (Papel oder Plaque), Juckreiz
• Lichen simplex chronicus Vidal: kein Schleimhautbefall, ausgeprägter Juckreiz, extragenitale kutane Läsionen
• Psoriasis anogenitalis: kein Schleimhautbefall, scharfe Begrenzung zur gesunden Haut, häufig extragenitaler Befall.
• Vitiligo: scharfe, häufig bizarre Abgrenzung zur gesunden Haut, kein Juckreiz, keine Induration der Haut, extragenitaler Befall!
• Candidainfektionen: mazerierte, unscharf begrenzte Areale, Leisten- und Analregion meist mit mitbefallen. Adipositas, Immobilität, Immunstörungen, mykologischer Nachweis der Hefen!

• Lichen ruber: Hyperorthokeratose, ausgeprägte Hypergranulose, keine Ausbildung einer sog. öden Zone
• Acrodermatitis chronica atrophicans: eher spärliches diffuses lymphozytäres Infiltrat, relvanter Anteil an Plasmazellen. Keine subepithelial bandförmige Anordnung.
• Mycosis fungoides: dichtes lymphozytäres Infiltrat, Zell-und Kernpolymorphie, eosinophile Granulozyten. Keine öde Zone.

Histologie

Frühstadium: Epithelatrophie, Orthohyperkeratose, Hypergranulose. Subepithelial bandförmig angeordnetes, epidermotropes rundzelliges Infiltrat. Dieses kann durchaus erhebliche Dichte annehmen, so dass differentialdiagnostisch eine Abgrenzung zu epidermotropen kutanen T-Zell-Lymphomen erfolgen muss.

Intermediärstadium: Elastikafaser-freie Ödemzone, mit weitklaffenden Blut- und Lymphgefäßen, die sich reißverschlussartig zwischen die Epidermis und die bandförmige Entzündungszone schiebt. Bullöse Umwandlung ist möglich.

Spätstadium: Zunehmende Sklerosierungstendenz und Rückbildung der zellulären Entzündungsphänomene.

Immunhistologie: Dermales Infiltrat überwiegend aus T-Lymphozyten, mit hohem Anteil von B-Lymphozyten (20-30%).

Hinweis: ausreichend tiefe Stanzbiopsie (nicht quetschen) aus läsionaler, nicht-steroidal vorbehandelter Haut/Schleimhaut.
